Sal’s Stars of the Week:

This week’s SALS star from the Resp Therapy & Perfusion departments goes to Kyle Nickart and Jean-Claude (J-C) Simard for their foresight and patient advocacy in the care of a status asthmaticus patient who was sent over St Boniface hospital for an ECMO consult.

While many may not be aware, care for status asthmaticus at St Boniface Hospital has significantly changed in the past few months thanks to capital investments and significant staff buy in. St Boniface RT department was supported with an adjunct to one of our ventilators which allows us to ventilate patients with status asthmaticus using a mix of helium and oxygen. In addition, SBH RT has also rolled out a continuous nebulization policy in the critical care units which allows for continuous delivery of concentrated Ventolin continuously without constant refilling of the medication. Recently also adding syringe pump management competency to the list

When this was rolled out, we soon received a patient from HSC for an ECMO consult following decompensation due to worsening status asthmaticus. With the foresight and courage to put education they just received to use, J-C preemptively placed the appropriate ventilator in the room prior to the patient coming and Kyle managed to pitch a case to the attending physician at the time to trial this therapy. This foresight eventually led to no indication for ECMO and the management of the patient with heliox and continuous nebulization until extubation and discharge from the ICMS. Both J-C and Kyle have been working at SBH since graduation and we are proud to have them as part of the team and for the continued bedside excellence!

Some Good News

We performed an EVH saphenous vein harvest January 13, 2023 for the first time on a young man in his 50’s who was a daily runner.  The support from the foundation for this opportunity will improve patients post op care and lives.

The incision was approximately one inch and I was able to retrieve 2 pieces of vein from the thigh.  Normally that would require an incision from the knee up to the groin.  Over time and experience we will be able to harvest vein from the ankle to groin with the same one inch incision. 

As a result, patients are able to ambulate quicker.  In centres where the leg infection rate is 17-20% the EVH system decreased the risk to less than 1%.

Well, tomorrow will be the next harvest.  My goal will be for the same success.
