The Heart Failure Pathway Referral Tool and Guideline Directed Medical Therapy (GDMT) webpages launched Feb. 3, 2022

The link can be accessed on this page (screenshot below):  https://cardiacsciencesmb.ca/health-professionals/clinical-areas-pathways/cardiac-clinics/heart-failure/ or the direct link to the tool is: https://cardiacsciencesmb.ca/pathway-referral-tool/.

Special congrats to Kyla Siatecki and Emily Hyde (and their teams) for all of their contributions to make this launch possible. The tool and content development included the input of patients and providers across multiple disciplines from across the continuum of care – acute and community. This is a  great Cardiac Sciences team effort which included experts from clinical and non-clinical areas and support from our vendor website design team!

Program Highlight of the Month

The Heart Failure Clinic at St Boniface Hospital cares for patients from Manitoba, Northwestern Ontario, and Nunavut with complex heart failure along with patients post-heart transplant and with ventricular assist devices.

The clinic is run by 3 MDs, 2 NPs, 4 RNs, including a transplant coordinator who provide specialized patient care and education. The clinic is also supported by various administrative staff and the research group.

The HF Clinic currently cares for over 1100 heart failure patients and this number is anticipated to grow as the number of patients living with heart failure continues to grow.

Sals Star of the Week:  Rose Grant (introduced by Tim Dyck)

Rose is recognized as an expert in the management of heart failure including rare conditions  such as amyloid cardiomyopathy.  She has shared her expertise on a local and national basis.  She previously received her certification in heart failure from the AAHFN. Rose is a strong advocate for patients living with heart failure. In addition, Rose has been instrumental in initiating and leading the nurse run clinics, where the heart failure nurses titrate and adjust medication’s under the supervision of an attending physician. Rose is very team oriented in her approach, and is the first person to volunteer to take on extra or cover for a colleague who is away. Recently Rose stepped in to take on extra clinic duties due to some unanticipated  gaps in our clinic coverage. It was much appreciated!

Rose is always going above and beyond for her patients, advocates for them always. She has been known to deliver meds to patients homes, has delivered food hampers to patients at Christmas, helped organize patient engagement activities and has even attended their funerals.

A bit about Rose outside of St. Boniface Hospital. She is a mother of two adult children, is a very physical and active person, has a lively social life, and is a great entertainer. She loves to travel. Is an avid Winnipeg Blue Bomber fan and tail-gater. She also enjoys several sports and including curling, and golf.

Our patients are fond of Rose and feel well supported by her clinical expertise and willingness to connect with them personally!

Please join me in celebrating Rose as our Sal’s star of the week!
